MARY'S CARECONNECT

1. Purpose

Mary's CareConnect is a care navigation and referral platform designed to help connect families, caregivers, seniors, and disabled adults with trusted providers and community resources. By joining the platform, Provider agrees to participate as a referral and community resource partner.

2. Provider Responsibilities

• •Maintain accurate business information
• •Provide truthful service descriptions
• •Respond to referrals in a timely manner
• •Comply with all applicable laws and regulations
• •Maintain any required licenses/certifications
• •Provide professional and ethical services
• •Notify Mary's CareConnect of major operational changes

Provider understands that referral opportunities are not guaranteed.

3. Platform Services

Mary's CareConnect may provide:

• •Referral opportunities
• •Provider profile visibility
• •Care navigation support
• •Referral tracking tools
• •Provider dashboard access
• •Analytics and reporting features

Platform reserves the right to update or modify services.

4. Membership & Fees

Provider may participate under one of the following structures:

• •Monthly Membership Plan
• •Per Referral / Per Client Plan
• •Promotional or Trial Plan

Membership pricing, referral fees, and future billing structures may be updated with notice.

5. Referral Tracking

Provider acknowledges that referrals generated through Mary's CareConnect may be tracked for:

• •Referral activity
• •Reporting
• •Quality assurance
• •Future billing purposes

Provider agrees not to intentionally bypass the platform for active tracked referrals generated through Mary's CareConnect.

6. Provider Status

Mary's CareConnect may manage provider participation using statuses including:

• •Active
• •Paused
• •Pending Approval
• •Suspended
• •Cancelled

Providers may pause participation voluntarily. Mary's CareConnect reserves the right to suspend or remove providers for: inaccurate information, unethical conduct, repeated non-response, complaints, or policy violations.

7. Accepting New Clients

Providers may control visibility using the "Accepting New Clients" toggle within the provider dashboard. When OFF: provider may be hidden from referrals/public listings and referrals may pause temporarily.

8. No Guarantee of Clients

Mary's CareConnect does not guarantee referral volume, client conversion, business revenue, or placement priority.

9. Independent Provider Status

Providers remain independent organizations and are not employees, agents, or representatives of Mary's CareConnect.

10. Data & Privacy

Provider agrees to protect confidential client information and use referral data only for legitimate service purposes.

11. Termination

Either party may terminate participation at any time. Mary's CareConnect reserves the right to suspend or terminate accounts for policy violations or misuse of the platform.
